SCHOLARSHIP PROGRAM

$1,000 Scholarship for the graduating class of 2025.

Star Community Health is a Federally Qualified Health Center-Look Alike, with thirteen practices and four mobile dental units, serving over 45,000 patients throughout the Lehigh Valley and Warren County NJ. Star Community Health is committed to providing the highest patient care and fostering community support. In celebration of our continued success in in providing quality care to all those in need, we are excited to offer the annual Star Community Health Scholarship.

HOW TO APPLY

For Allentown, Bethlehem, Wilson, Easton and Phillipsburg Area School District Seniors

Application to be submitted by March 15, 2025 by 4 pm to Ellen.Denizard@starcommunityhealth.org:

  • The application needs to include
    • Current High School
    • College/University you will attend fall 2025
    • Intended major/area of study and future career goals and why you plan to pursue them.
    • Essay (1-2 pages) on how education affects the long-term health of an individual and community
  • Indicate your availability to participate in a required observation experience 6/23-6/25 or 6/30-7/2 if you are selected.
  • Letter of recommendation (1) from a teacher or community leader.

Selected students must complete the following by May 15, 2025 in order to receive the scholarship:

  • Complete and submit required forms:
    • W9 Form (required for scholarship payment)
    • Proof of immunizations
    • Proof of COVID vaccination or attestation of having had COVID infection
    • HIPAA Confidentiality form
    • Attend a 3-day Shadow Experience during one of these timeframes: 6/23-6/25 or 6/30-7/2
