We offer assistance in accessing various resources through our Social Worker, Financial Counselor and Outpatient Care Manager. We can perform many point-of-care tests at the time of your visit including, EKG’s, Hemoglobin A1C, blood glucose fingerstick, diabetic eye exams, urine dip, spirometry and hearing/vision tests.

We offer assistance in accessing various resources through our Social Worker, Financial Counselor and Outpatient Care Manager. In addition, we have a Certified Spanish Interpreter on site to help facilitate optimal communication with patients, care and management of diabetes and other chronic illnesses, including transitioning from hospital to home through specialized office visits. We can perform many point-of-care tests at the time of your visit including, EKG’s, Hemoglobin A1C, blood glucose fingerstick, diabetic eye exams, urine dip, spirometry and hearing/vision tests.

A provider is available 24 hours a day to provide Star Community Health patients with access to needed care. To reach a provider after hours, please call your doctor’s office and the answering service can page the provider on call.
